How to Request Appointments:
Our office is available to schedule appointments during the hours listed. We will do our best to schedule an appointment that is convenient for you. We also offer an Online Scheduling service, if that fits your needs. Book an Appointment with us today!
No Show Notice
If for any reason you need to cancel your appointment, cancellations must be called in 24 hours. If no show, no call there will be a $50.00 fee.

Financial Responsibility
You are financially responsible for all services provided by our practice. These costs can include:
Co-payments / Co-insurance
Deductibles
Services not covered by your insurance plan
Balance due on accounts
Payments can be made at the office or via mail. We accept cash, personal checks, traveler's checks, Visa, and MasterCard
Prescription Refills
Please remind us at your next office visit if you need a refill on prescriptions. If you need a refill prior to that time, call your pharmacy and ask the pharmacy to fax us a prescription refill request form to (815) 436-1656.
Insurances we accept
Please bring in your insurance card to all of your visits and fill out our validation form before coming to your visit at farahturkmd.com/validation. Here we ask for you to attach a copy of your Driver's License and Insurance Card before your appointment time, we also have an iPad available in the office to digitality upload your cards to our portal.
